This variant is a semi-conservative substitution in which a neutral polar amino acid is replaced with a positive polar one, altering a position that is well conserved throughout evolution and is located in the Lever domain (Lutzen 2008). Multiple in silico algorithms predict that this variant may be damaging to protein structure and function. Based on the currently available information, we consider MSH2 Gln374His to be a variant of uncertain significance.

The glutamine at codon 374 is replaced by histidine, an amino acid with highly similar properties. In one study, this variant was reported in 1/60,466 breast cancer cases and in 2/53,461 controls (Dorling et al. N Engl J Med. 2021 02;384:428-439). In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was reported to be functionally neutral (Jia X et al. Am J Hum Genet, 2021 Jan;108:163-175). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
