This alteration results from a C to T substitution at nucleotide position 737. The proline at codon 246 is replaced by leucine, an amino acid with similar properties. This alteration has been detected in multiple individuals meeting clinical diagnostic criteria for PTEN-hamartoma tumor syndrome (PHTS), and was demonstrated to occur de novo in a 2 year-old boy with macrocephaly and developmental delay (Marsh D et al. Hum Mol Genet. 1999;8(8):1461-1472; Otto LR et al. Am. J. Med. Genet. 1999 Mar;83:3-5; Hobert J et al. Genet Med. 2012 Jun;14(6):616-9; Zhou X et al. Am J Hum Genet. 2003 Nov;73(5):1191-1198; Mester J and Eng C. Genet. Med. 2012 Sep;14(9):819-22; Vanderver A et al. Am. J. Med. Genet. A, 2014 Mar;164A:627-33; Chen HH et al. J. Allergy Clin. Immunol. 2016 Jun). A yeast-based assay demonstrated this this alteration combined with another missense alteration failed to rescue yeast cells from p110alpha CAAX induced growth inhibition and release GFP-AKt1 from the cellular membrane, although both alterations by themselves had similar activity to wildtype. The authors postulated that this assay suggests patient-dependent loss of function effects (Andrés-Pons A et al. Cancer Res. 2007 Oct;67:9731-9). In addition, p.P246L has been described as a pathogenic mutation leading to impaired PTEN protein levels and signaling in Lhermitte-Dulcose disease cells (Zhou XP et al. Am. J. Hum. Genet. 2003 Nov;73:1191-8). Based on protein sequence alignment, this amino acid position is highly conserved in available vertebrate species. Given the available evidence, this alteration is classified as a pathogenic mutation.

This variant has been observed in individuals with Bannayan-Riley-Ruvalcaba syndrome and has been reported as de novo in a child presenting with vascular anomalies and macrocephaly (Otto 1999, Hobert 2012, Allen 2013). Yeast-based assays studying the functional impact of PTEN Pro246Leu by Rodriguez-Escudero et. al. (2011) found growth rescue comparable to wild-type but a significant impact on phosphatase activity. Likewise, Zhou et al. (2003) reported weak PTEN expression and elevated P-Akt levels in a dysplastic gangliocytoma from an individual harboring this variant in their germline. PTEN Pro246Leu was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Since Proline and Leucine differ in some properties, this is considered a semi-conservative amino acid substitution. PTEN Pro246Leu occurs at a position that is conserved across species and is located in the C2 domain (Wang 2008). Based on current information, we consider this variant to be pathogenic.

In addition, it was reported as a de novo occurrence in an infant with infantile spasms and clinical features of a PTEN hamartoma tumor syndrome ( Allen 2013). Functional studies provide some evidence that the p.Pro246Leu varia nt is associated with decreased protein activity (Rodriguez-Escudero 2011). This variant was absent from large population studies. In summary, although addition al studies are required to fully establish its clinical significance, the p.Pro2 46Leu variant is likely pathogenic.

BRRS is allelic to Cowden syndrome and is characterized by macrocephaly, hamartomatous intestinal polyposis, lipomas, and pigmented macules of the glans penis. The variant was further detected de novo in a cohort of 264 trio diagnosed with seizure. The patient had infantile spasms in addition to vascular anomalies and macrocephaly [PMID 23934111]. We have observed this variant, de novo, in a patient with autism in our internal database. This c.737C>T (p.Pro246Leu) variant was not observed in the gnomAD population database. Proline at amino acid position 246 is conserved in mammals. While not validated for clinical use, computer-based algorithms SIFT and Polyphen2 predict this p.Pro246Leu to be deleterious. This variant is thus classified as pathogenic.
Invitae RCV000613031 SCV001223839 pathogenic PTEN hamartoma tumor syndrome 2020-09-20 criteria provided, single submitter clinical testing This sequence change replaces proline with leucine at codon 246 of the PTEN protein (p.Pro246Leu). The proline residue is highly conserved and there is a moderate physicochemical difference between proline and leucine. This variant is not present in population databases (ExAC no frequency). This variant has been observed in individuals affected with symptoms consistent with PTEN hamartoma tumor syndrome (PHTS) (PMID: 22261759, 27477328, 24375884), and to segregate with disease in a single family (PMID: 10076877). In addition, it has been observed to be de novo in one individual affected with PHTS (PMID: 23934111). This variant is also known as chr10:89717712 C>T in the literature. ClinVar contains an entry for this variant (Variation ID: 142269). This variant has been reported to have conflicting or insufficient data to determine the effect on PTEN protein function (PMID: 21828076, 14566704, 17942903). For these reasons, this variant has been classified as Pathogenic.
