Labcorp Genetics (formerly Invitae), Labcorp RCV001377819 SCV001575254 pathogenic RYR1-related disorder 2023-11-13 criteria provided, single submitter clinical testing This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 2234 of the RYR1 protein (p.Arg2234Cys). This variant is present in population databases (rs201465595, gnomAD 0.009%). This missense change has been observed in individual(s) with autosomal dominant RYR1-related conditions as well as individuals without clinical features of disease (PMID: 24195946; Invitae). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 224389). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RYR1 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.

Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with malignant hyperthermia susceptibility in the literature. This variant has been identified in 5/251106 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

This variant was reported in a large exome sequencing cohort of individuals not ascertained for malignant hyperthermia susceptibility (supplemental digital content 3, Gonsalves et al. 2013. PubMed ID: 24195946). This variant is reported in 0.0087% of alleles in individuals of Latino descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
