Laboratory for Molecular Medicine, Mass General Brigham Personalized Medicine RCV000039855 SCV000063546 uncertain significance not specified 2013-11-08 criteria provided, single submitter clinical testing Variant classified as Uncertain Significance - Favor Benign. The Asp3500Glu vari ant in TTN has now been reported by our laboratory in 2 adults with DCM. This va riant has been identified in 0.1% (7/8212) of European American chromosomes by t he NHLBI Exome Sequencing Project (http://evs.gs.washington.edu/EVS/; dbSNP rs55 906845). Aspartic acid (Asp) at position 3500 is conserved in mammals, but not i n lower species with several fish species carrying the variant amino acid (gluta mic acid), raising the possibility that this change may be tolerated. Additional computational analyses (amino acid biochemical properties, AlignGVGD, SIFT, Pol yPhen-2) suggest that this variant may not impact the protein. Although this dat a supports that the Asp3500Glu variant may be benign, additional studies are nee ded to fully assess its clinical significance.

Illumina Laboratory Services, Illumina RCV001132041 SCV001291692 benign Tibial muscular dystrophy 2018-01-13 criteria provided, single submitter clinical testing This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score and internal cut-off values, a variant classified as benign is not then subjected to further curation. The score for this variant resulted in a classification of benign for this disease.

Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease.

ARUP Laboratories, Molecular Genetics and Genomics, ARUP Laboratories RCV000185316 SCV001472237 uncertain significance not provided 2019-09-25 criteria provided, single submitter clinical testing The TTN c.14232C>A; p.Asp4744Glu variant (rs55906845; ClinVar Variation ID: 46585) is rare in the general population (<1% allele frequency in the Genome Aggregation Database) and has not been reported in the medical literature in association with dilated cardiomyopathy (DCM) or other TTN-related disease. The clinical relevance of rare missense variants in this gene, which are identified on average once per individual sequenced in affected populations (Herman 2012), is not well understood. Yet, evidence suggests that the vast majority of such missense variants do not contribute to the clinical outcome of DCM (Begay 2015). Thus, the clinical significance of the p.Asp4744Glu variant cannot be determined with certainty. References: Begay RL et al. Role of Titin Missense Variants in Dilated Cardiomyopathy. Women's Health and Genetics/Laboratory Corporation of America, LabCorp RCV000039855 SCV001774769 likely benign not specified 2021-07-19 criteria provided, single submitter clinical testing Variant summary: TTN c.10500C>A (p.Asp3500Glu) results in a conservative amino acid change located in the I-band region of the encoded protein sequence. Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.00023 in 248492 control chromosomes, predominantly at a frequency of 0.00047 within the Non-Finnish European subpopulation in the gnomAD database. The observed variant frequency within Non-Finnish European control individuals in the gnomAD database is approximately 1.2 fold of the estimated maximal expected allele frequency for a pathogenic variant in TTN causing Dilated Cardiomyopathy phenotype (0.00039), strongly suggesting that the variant is a benign polymorphism found primarily in populations of Non-Finnish European origin. The variant, c.10500C>A, has been reported in the literature in an individual affected with Sudden Unexplained Death (Campuzano_2015), however without strong evidence for causality. This report therefore does not provide unequivocal conclusions about association of the variant with Dilated Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Seven clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, and classified the variant as likely benign (n=1) or VUS (n=6). Based on the evidence outlined above, the variant was classified as likely benign.

GeneDx RCV000185316 SCV000238194 not provided not provided 2014-06-23 no assertion provided clinical testing Missense variants in the TTN gene are considered 'unclassified' if they are not previously reported in the literature and do not have >1% frequency in the population to be considered a polymorphism. Research indicates that truncating mutations in the TTN gene are expected to account for approximately 25% of familial and 18% of sporadic idiopathic DCM; however, truncating variants in the TTN gene have been reported in approximately 3% of reported control alleles. There has been little investigation into non-truncating variants. (Herman D et al. Truncations of titin causing dilated cardiomyopathy. N Eng J Med 366:619-628, 2012) The variant is found in DCM,CARDIOMYOPATHY panel(s).
