The Val9435Ile vari ant in TTN has not been previously identified by our laboratory or in the litera ture, but has been identified in 1/8182 of European American chromosomes by the NHLBI Exome Sequencing Project (http://evs.gs.washington.edu/EVS/; dbSNP rs36993 2282). Valine (Val) at position 9435 is not well conserved in evolution and pand a carries an isoleucine (Ile) at this position, raising the possibility that thi s change may be tolerated. Additional computational analyses (biochemical amino acid properties, AlignGVGD, and PolyPhen2) also suggest that the Val9435Ile vari ant may not impact the protein, though this information is not predictive enough to rule out pathogenicity. In summary, although the available data raise the po ssibility that the Val9435Ile variant may be benign, additional studies are need ed to fully assess its clinical significance. |

Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4.9e-05 in 243346 control chromosomes, predominantly at a frequency of 9e-05 within the Non-Finnish European subpopulation in the gnomAD database. However, in certain European subpopulations the variant occurs with an even higher frequency, e.g. in North-western Europeans (allele frequency: 0.00017). This frequency somewhat lower than the maximum expected for a pathogenic variant in TTN causing Dilated Cardiomyopathy (0.00039), allowing no clear conclusions about variant significance. To our knowledge, no occurrence of c.28303G>A in individuals affected with Dilated Cardiomyopathy and no experimental evidence demonstrating its impact on protein function have been reported. Two clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. One laboratory classified the variant as likely benign, and the other laboratory classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as VUS-possibly benign. |
