RCV001130197 | SCV001289767 | likely benign | Myopathy, myofibrillar, 9, with early respiratory failure | 2018-01-13 | criteria provided, single submitter | clinical testing | This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score and internal cut-off values, a variant classified as likely benign is not then subjected to further curation. The score for this variant resulted in a classification of likely benign for this disease. |

RCV001130198 | SCV001289768 | uncertain significance | Dilated cardiomyopathy 1G | 2018-01-13 | criteria provided, single submitter | clinical testing | This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. |

RCV001328447 | SCV001519587 | uncertain significance | not specified | 2021-03-01 | criteria provided, single submitter | clinical testing | Variant summary: TTN c.55922G>A (p.Arg18641Gln) results in a conservative amino acid change located in the A-band of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8.1e-05 in 247812 control chromosomes. This frequency is not significantly higher than expected for a pathogenic variant in TTN causing Dilated Cardiomyopathy (8.1e-05 vs 0.00039), allowing no conclusion about variant significance. To our knowledge, no occurrence of c.55922G>A in individuals affected with Dilated Cardiomyopathy and no experimental evidence demonstrating its impact on protein function have been reported. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. Multiple laboratories reported the variant with conflicting assessments. Based on the evidence outlined above, the variant was classified as uncertain significance. |

RCV000714067 | SCV001549758 | uncertain significance | not provided | no assertion criteria provided | clinical testing | The TTN p.Arg18641Gln variant was not identified in the literature but was identified in dbSNP (ID: rs148684589), ClinVar (classified as uncertain significance by Athena Diagnostics and Ambry Genetics), and LOVD 3.0 (variant effect not shared). The variant was identified in control databases in 28 of 279170 chromosomes at a frequency of 0.0001003 (Genome Aggregation Database March 6, 2019, v2.1.1). The variant was observed in the following populations: African in 13 of 24174 chromosomes (freq: 0.000538), Other in 3 of 7086 chromosomes (freq: 0.000423), European (non-Finnish) in 10 of 127526 chromosomes (freq: 0.000078), East Asian in 1 of 19264 chromosomes (freq: 0.000052) and South Asian in 1 of 30582 chromosomes (freq: 0.000033), but was not observed in the Latino, Ashkenazi Jewish, or European (Finnish) populations. The p.Arg18641 residue is conserved in mammals and four of five computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) suggest that the variant may impact the protein; this information is not predictive enough to assume pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. |
