GeneDx RCV000480435 SCV000570388 uncertain significance not provided 2024-02-12 criteria provided, single submitter clinical testing In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Has not been previously published as pathogenic or benign to our knowledge
Illumina Laboratory Services, Illumina RCV001110309 SCV001267730 uncertain significance Senior-Loken syndrome 6 2018-01-12 criteria provided, single submitter clinical testing This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease.

Labcorp Genetics (formerly Invitae), Labcorp RCV001247923 SCV001421376 uncertain significance Familial aplasia of the vermis; Meckel-Gruber syndrome; Nephronophthisis 2022-10-24 criteria provided, single submitter clinical testing This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 1729 of the CEP290 protein (p.Arg1729Trp). This variant is present in population databases (rs201353893, gnomAD 0.03%). This missense change has been observed in individual(s) with clinical features of retinitis pigmentosa (Invitae). ClinVar contains an entry for this variant (Variation ID: 421248). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CEP290 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Fulgent Genetics, Fulgent Genetics RCV002489161 SCV002777388 uncertain significance Leber congenital amaurosis 10; Meckel syndrome, type 4; Senior-Loken syndrome 6; Joubert syndrome 5; Bardet-Biedl syndrome 14 2022-04-19 criteria provided, single submitter clinical testing
Ambry Genetics RCV004023149 SCV004926236 uncertain significance Inborn genetic diseases 2022-01-18 criteria provided, single submitter clinical testing The c.5185C>T (p.R1729W) alteration is located in exon 38 (coding exon 37) of the CEP290 gene. This alteration results from a C to T substitution at nucleotide position 5185, causing the arginine (R) at amino acid position 1729 to be replaced by a tryptophan (W). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.

Department of Pathology and Laboratory Medicine, Sinai Health System RCV000480435 SCV001551633 uncertain significance not provided no assertion criteria provided clinical testing The CEP290 p.Arg1729Trp variant was not identified in the literature nor was it identified in Cosmic or LOVD 3.0. The variant was identified in dbSNP (ID: rs201353893) and ClinVar (variant is classified as VUS by GeneDx). The variant was identified in control databases in 47 of 277796 chromosomes at a frequency of 0.000169 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: European (non-Finnish) in 38 of 126646 chromosomes (freq: 0.0003), African in 5 of 24066 chromosomes (freq: 0.000208), Latino in 3 of 35014 chromosomes (freq: 0.000086) and South Asian in 1 of 30438 chromosomes (freq: 0.000033); it was not observed in the Ashkenazi Jewish, East Asian, European (Finnish) and Other populations. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. The p.Arg1729 residue is conserved in mammals but not in more distantly related organisms, and four out of five computational analyses (PolyPhen-2, SIFT, BLOSUM, MutationTaster) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance.
PreventionGenetics, part of Exact Sciences RCV004527592 SCV004106880 uncertain significance CEP290-related disorder 2024-04-22 no assertion criteria provided clinical testing The CEP290 c.5185C>T variant is predicted to result in the amino acid substitution p.Arg1729Trp. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.030% of alleles in individuals of European (Non-Finnish) descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
