Illumina Laboratory Services, Illumina RCV000261978 SCV000381336 uncertain significance Leber congenital amaurosis 10 2018-01-13 criteria provided, single submitter clinical testing This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease.

Labcorp Genetics (formerly Invitae), Labcorp RCV002519085 SCV003243515 uncertain significance Familial aplasia of the vermis; Meckel-Gruber syndrome; Nephronophthisis 2022-10-16 criteria provided, single submitter clinical testing This sequence change replaces isoleucine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 2000 of the CEP290 protein (p.Ile2000Val). This variant is present in population databases (rs183071230, gnomAD 0.03%). This missense change has been observed in individual(s) with CEP290-related disease (Invitae). ClinVar contains an entry for this variant (Variation ID: 281249). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt CEP290 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

Department of Pathology and Laboratory Medicine, Sinai Health System RCV000313250 SCV001552541 uncertain significance not provided no assertion criteria provided clinical testing The CEP290 p.Ile2000Val variant was not identified in the literature but was identified in dbSNP (ID: rs183071230), ClinVar (classified as uncertain significance by Illumina, EGL Genetic Diagnostics, and Fulgent Genetics), and LOVD 3.0 (variant effect not shared). The variant was identified in control databases in 47 of 273358 chromosomes at a frequency of 0.0001719 (Genome Aggregation Database March 6, 2019, v2.1.1). The variant was observed in the following populations: European (non-Finnish) in 44 of 125324 chromosomes (freq: 0.000351), South Asian in 2 of 28942 chromosomes (freq: 0.000069) and Latino in 1 of 34100 chromosomes (freq: 0.000029), but was not observed in the African, Ashkenazi Jewish, East Asian, European (Finnish), or Other populations. The p.Ile2000 residue is conserved in mammals but not in more distantly related organisms however four out of five computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance.
PreventionGenetics, part of Exact Sciences RCV004535253 SCV004119819 uncertain significance CEP290-related disorder 2023-12-20 no assertion criteria provided clinical testing The CEP290 c.5998A>G variant is predicted to result in the amino acid substitution p.Ile2000Val. This variant was reported in an individual with nephronophthisis-associated ciliopathies and in an individual with sperm motility disorders; however, no additional studies were performed to help assess the pathogenicity of this variant (Table S5, Halbritter et al. 2012. PubMed ID: 23188109; Oud et al. 2021. PubMed ID: 34089056). This variant is reported in 0.035% of alleles in individuals of European (Non-Finnish) descent in gnomAD, which may be too common to be a primary cause of disease. Although we suspect that this variant may be benign, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
